A new tax form – 1095-C

Children’s Hospital of Wisconsin is required by federal health care reform law to provide our benefit-eligible employees with the new IRS Form 1095-C so that you can report your enrollment in a Children’s health plan for tax purposes. Form 1095-C is a new tax form, similar to a W-2 form. It will be provided on Employee Self Service in February 2016 if you elect to receive it electronically. Otherwise, it will be mailed home. More information on the electronic consent and Form 1095-C will come through email and Newsbreak in early February. While we must provide you Form 1095-C to verify your 2015 health plan enrollment, you can file your 2015 taxes without it.
